Easy to Use Health Passport Form
The FCIC Health Passport Form (available in both English and Spanish) are wonderful tools that help communicate needs between caregivers. Please print them out and use them when you need to! They are an excellent tool to help alleviate questions and confusion when meeting a doctor or other caregiver for the first time.
